Purpose of Job: Improve the overall health of the communities we serve by providing individualized nutritional support to patients for their particular situation as follows:
Essential Duties and Responsibilities:
- Meets directly with patient to identify dietary needs through nutritional assessment based on nutrition-focused physical finings and patient history.
- Provides nutritional counseling, identifying goals and supporting education with evidence-based care.
- Develops detailed care plans and documents as part of the electronic health record in accordance with established program standards.
- Assesses plans and implements the delivery of medical nutrition therapies in collaboration with the patients’ inter-disciplinary team
- Monitors and evaluates patient’s progress trough routine nutrition interventions to support positive health outcome.
- Participates and supports patients in various program committees; including developing educational programs for various ethnic, cultural, and social groups to teach nutrition education.
- Remains up to date on evidence-based research and recommendations ways to expand and improve nutrition program.
- Performs other duties assigned by doctors.
Qualifications:
Education/Experience:
Bachelor’s Degree in Nutrition or related field required. Registered Dietitian credentialed with Commission on Dietric Registration and eligible for state licensure. Experience in Chronic disease including HIV and Diabetes and/ or maternity nutrition counseling preferred.
Skills:
Excellent oral and written communication skills required.
Computer skills required. BLS (CPR/AED) required. Commitment to supporting a safe, respectful, equitable, and inclusive environment required.
